Temporary Guardianship Agreement (Parental Guardianship of Minor)
By completing and having notarized The Parental Guardianship Agreement, also known as Parental Guardianship of Minor or Child, the parent(s) / legal guardian(s) of a minor child, can grant temporary guardianship status of their child(ren) to an adult of their choice. This does not relinquish birth-right parental / custodial rights of the child’s (or children’s) parents. It simply allows the parent(s) to appoint a temporary guardian of their child(ren) in their absence. This form is often used by parent(s) who may be away from their child(ren) for extended periods of time. This form is customarily signed and notarized by notary public.
